Prescription Collection Consent Forms
If you are registered with a local Doctor's Surgery we can collect your repeat prescriptions from the surgery and dispense your medication for you, ready for collection.
We aim to have all prescriptions ready for collection within 48 hours of you ordering the prescription from the doctor's surgery.
If you would like to use this free service please complete the consent form below and return it to the pharmacy or please ask at the pharmacy counter to provide you with a paper copy for you to complete.

To ensure you get the best results from your medication and that the medicine is suitable for you, the pharmacy staff may ask selected questions before selling or providing advise on products. All of our healthcare assistants are properly trained using accredited courses so they can advice you about treating common ailments.
